Athlete Information Letter
This letter describes what to do with the following forms.

Athlete Insurance Questionnaire
This gathers information about the student-athletes personal or family insurance.

Initial Athlete Physical Form
This form is only for athletes who are new to NIC and have not had a physical exam as an NIC athlete.

Repeat Athlete Physical Form
This form is only for athletes who have had a physical exam as an NIC athlete.

Student-Athlete Authorization to Release Records
This form allows the student-athlete to select which records NIC can release and the person or organization allowed to receive the information.

Athlete Commitment to Class
This form explains the academic expectations of the student-athlete and asks the student-athlete to acknowledge he/she has read and understands the form.

Athlete Acknowledment of Insurance Liability Form
This form is a waiver stating the student-athlete and his/her parent or guardian understand the insurance provided by NIC Athletics is secondary to their family insurance and that the ultimate responsibility of payment for any medical service is the student-athlete's.

Awareness of Risk/Waiver and Release
This form explains the risks of participating in athletics and asks the student-athlete to acknowledge he/she has read and understands the form.

Student-Athlete Information Sheet
This letter describes what the athlete should do when an injury occurs.

Student-Athlete Rights
This letter describes the steps to be followed if you believe you have been treated unfairly or if you have a complaint or grievance with a coach, financial aid decision, or medical/insurance coverage.
